This program offers support and case management to families who care for people with moderate to severe disabilities and high support needs.
- Families caring for a person aged 6-64 years with an intellectual, physical or sensory disability, or acquired brain injury residing in the Lakes Entrance, Nowa Nowa and Lake Tyers Aboriginal Trust.
- People not eligible include those with: Attention Deficit and Associated Disorders; chronic disease; psychiatric disability; families in receipt of Family Options Funding.
Monday - Friday: 8.30 am > 5.00 pm

This program provides case coordination and flexible care packages to families of children with a diagnosed disability that has complex behavioural needs. The Service provides information and links to other services to support families to care for their children. Brokerage money may be available to access specialist services.
Referrals to be made through Specialist Children's Services at DHS, Traralgon.
- Families of children aged 0-school entry age, with development delay / global delay.
- People not eligible include those with: Attention Deficit and Associated Disorders; chronic disease; psychiatric disability; families in receipt of Family Options Funding.
Wednesday, Thursday and alternate Fridays: 8.30 am > 5.00 pm
