Health Assessments



Health assessments provide the opportunity for a GP to undertake an in-depth assessment in an individual’s health in order to ensure that optimal health and independence is maintained.  The assessment covers medical, physical, psychological and social aspects of health.  The initial appointment which generally takes place in the client’s home takes around two hours and is conducted by a registered nurse.  This is then followed up by at least a double appointment with the individuals GP at the clinic.



Who can use the service?

All people aged 75 and over or 15 to 59 years for Aboriginal and Torres Strait Islander people, who are living in the community or in a hostel level aged care facility.



Hours of Opening:

Monday - Friday: 9.00 am > 12.00 pm and 2.00 pm > 3.00 pm (nursing visit)

Monday - Friday: 9.00 am > 12.00 pm and 2.00 pm > 5.00 pm (general practitioner appointment)
