The Chronic Disease Management Clinic at the Koorie Community Centre provides additional and specific health services and support to all members of the Lakes Entrance Koorie community. The Clinic focuses on chronic disease prevention and management, and acknowledges the spiritual and cultural wellbeing of indigenous people, not just physical problems.

The Clinic is staffed by a General Practitioner along with the support of registered nursing support and Aboriginal Health Workers.
