People accessing this service must be referred to Gippsland Lakes Community Health by Department of Education and Early Childhood Development – Specialist Children’s Services.
This program provides case coordination and flexible care packages to families of children with a diagnosed disability that has complex behavioural needs. The Service provides information and links to other services to support families to care for their children. Brokerage money may be available to access specialist services.
Who can use the service?
- Families of children aged 0-school entry age, diagnosed with a disability or with development delay / global delay.
