The Home and Community Care Program provides a range of services to assist frail, older people and people with disabilities and their carers, to maintain independent living at home. An assessment of individual needs occurs prior to the commencement of service. The Home and Community Care team work closely with the District Nursing service.
Services include domestic assistance, home maintenance, personal care and respite.
Domestic Assistance A basic cleaning service and other day to day assistance including:
Home Maintenance Minor maintenance tasks and modifications are carried out around the home by a qualified worker. The home maintenance worker is also able to offer advice if the job is beyond the scope of the service, through referral to local tradespeople. Examples include:
Personal Care Personal care assists clients with tasks such as showering, dressing, toileting and feeding as well as meal preparation. Respite Respite care enables people caring for dependent adults or children to take a break, where the care demands are tiring and stressful.
Minimal fees apply based on a client's income level, and are determined after an assessment with a Home and Community Care Assessment officer.
