Gippsland Lakes Community Health provides a comprehensive diabetes care service. Qualified diabetes educators in collaboration with general practitioners and other health care professionals provide diabetes education and individual diabetes management plans as required.

Clients are reviewed regularly to ensure optimum diabetes care is achieved.

Who can use the service?

People with any form of diabetes or related conditions.
Family members of people with diabetes.
People interested in learning more about the disease.

Cost

As per Department of Human Services Community Health Fee Guidelines.
