People accessing this service must be referred to Gippsland Lakes Community Health by Department of Human Services – Intake and Response.
This program offers support and case management to families who care for people with moderate to severe disabilities and high support needs.

Who can use the service?

  • Families caring for a person aged 6-64 years with an intellectual, physical or sensory disability, or acquired brain injury residing in East Gippsland
  • People not eligible include those with: Attention Deficit and Associated Disorders; chronic disease; psychiatric disability; families in receipt of Family Options Funding.
